Salary: £28,577 (BP Band 5)

Hours: Full-time (37 hrs/week)

Contract: Permanent, with optional funded qualification (up to Level 6)

Base: Hybrid – Community, Office & Home (Cheshire & Warrington-wide)

We’re looking for a compassionate, well-organised individual to join our team as our TransPositive Link Worker, supporting trans and non-binary people across Cheshire and Warrington.

This is a frontline role working directly with individuals to connect them to non-clinical services that support their wellbeing – from peer support groups to housing, mental health services, social activities and more.

You’ll work closely with our NHS partners, local authorities and LGBTQ+ organisations with close links to Merseycare’s Transcend Social Prescribing and Community Engagement Service and Sahir.

The ideal candidate will have:

    • A working knowledge of social prescribing

    • Confidence supporting people one-to-one and in groups

    • Excellent organisational skills

    • The ability to work independently

    • A full UK driving licence and access to a vehicle
